Smithtown, NY 11787 –St. Catherine of Siena Nursing & Rehabilitative Care Center is honored to receive a 5-Star rating from the Centers for Medicare & Medicaid Services.  This rating is based on State Survey Agencies that perform initial surveys and periodic resurveys of all providers. These surveys are conducted to ascertain whether a provider/supplier meets applicable requirements for participation in the Medicare and/or Medicaid programs, and to evaluate performance and effectiveness in rendering a safe and acceptable quality of care. At St. Catherine of Siena Nursing & Rehabilitation Care Center we strive to deliver the highest quality of care to our residents and patients through our Centers of Excellence.   We take pride in knowing our facility is safe and our residents and family members feel secure. “I am very happy at St. Catherine of Siena because everything I need is taken care of and I feel secure here.
